The federal floor comes from the Child Care and Development Fund rules that apply to providers serving subsidized children, and most states write their licensing rules around them. Staff need pre-service or orientation training that covers prevention of sudden infant death syndrome and safe sleep practices, pediatric first aid and CPR, recognition and reporting of child abuse and neglect, medication administration, emergency preparedness, and more (45 CFR 98.41(a)(1)(i)). Every staff member needs a comprehensive background check, repeated at least once every five years (45 CFR 98.43).

What differs by state is the cycle. Pediatric CPR and first aid cards are issued for two years by the major providers. Mandated reporter training, safe sleep training, and annual in-service hours renew on cycles the state licensing agency sets, and a director's credential and a Child Development Associate credential renew on their own schedules. A background clearance done in year one is due again by year five, and it is easy to forget because nothing about daily work reminds anyone.
