The problem

The medical card expires on a Tuesday. The load was booked Monday.

DOT medical certificates run one to two years and every driver's cycle is different. Endorsements renew on their own schedules. A dispatcher can only check what is in front of them, and a card that lapsed last week does not announce itself until a roadside inspection or an insurance audit does.

What the rules require

Interstate commercial drivers must be medically examined and certified at least every 24 months (49 CFR 391.45). The medical examiner's certificate is valid for up to 24 months, and the examiner can issue it for less when a condition needs watching; FMCSA notes that high blood pressure and heart disease are commonly certified for one year. So a fleet of forty drivers has forty different expiration dates, and some of them are shorter than the driver expects.

For CDL holders the card is tied to the license. The driver has to keep the certificate on file with the state licensing agency. When it expires, the driver's medical certification status becomes "not certified", and the state must downgrade the CDL within 60 days (49 CFR 383.73(o)(4)). A lapsed card is not just a driver you cannot dispatch today. Left alone, it is a driver who loses the CDL itself and has to requalify to get it back.

Your driver qualification file has to hold the current certificate as well, so the same date matters twice: once for the driver's license and once for your audit. Endorsements run on their own clocks. The hazardous materials endorsement depends on a TSA security threat assessment that has to be renewed, and each state sets its own CDL renewal period. Track the medical card, the license, and each endorsement as separate records, and dispatch sees the earliest of them.
